沙漠干热与常温环境创伤失血性休克猪继发性肾损伤与TNF-alpha和IL-1beta变化的研究

中文摘要目的对比研究沙漠干热环境创伤失血性休克(THS)猪继发性肾损伤与炎症因子肿瘤坏死因子-alpha(TNF-alpha)和白细胞介素-1beta(IL-1beta)的变化关系。方法健康长白仔猪48头,随机分成干热实验组(DHS)及常温实验组(NTS), DHS组在热环境中暴露3 h后建立THS模型,模型成功后根据不同时间点分组为:T_1(模型成功后50 min组,n = 6)、T_2(模型成功后100 min组,n= 6)、T_3(模型成功后150 min组,n = 6) ;NTS组在常温环境中暴露3 h并且建立模型后分组为:T,(模型成功后50 min组,n = 6)、T_2(模型成功后100 min组,n = 6)、T_3(模型成功后150 min组,n = 6)、T4 (休克模型成功480 min组,n = 6)、T5(休克模型成功600 min组,n = 6)。各组在相应时间点收集血液检测血尿素氮和肌酐;取肾组织检测肾组织炎症因子TNF-alpha和IL-1beta ;取肾组织行肾小球及肾小管形态学检查及Paller评分。结果在T1时间点NTS组和DHS组血尿素氮和肌酐差异无统计学意义(P>0.05),在T_2和T_3时间点差异具有统计学意义(P<0.05);在T1、T_2和T_3时间点,DHS组Paller评分、炎症因子TNF-alpha和IL-1beta的表达量均高于NTS组(P<0.05)。相关分析发现,TNF-alpha与Paller评分呈正相关(r常温组= 0.863,r干热组=0.905,P<0.01) ;IL-1beta与Paller评分呈正相关(r常温组= 0.823,r干热组= 0.946,P<0.01)。同时DHS组T_3时间点血尿素氮、肌酐、Paller评分、TNF-alpha和IL-1beta 含量均高于NTS组T5时间点(P<0.05)。结论当干热环境合并THS时,肾损伤指标(血尿素氮和肌酐)、Paller评分和炎症因子(TNF-alpha及IL-1beta)较常温THS时明显升高,提示干热环境和THS共同驱动炎症因子表达在继发性肾损伤中可能起重要作用。
英文摘要Objective To research the relationship between the secondary renal injury and the change of inflammatory cytokines TNF-alpha and IL-1beta in dry-heat and normal temperature environment. Methods 48 healthy Landrace pigs were randomly divided into the dry-heat experimental (DHS) group and normal temperature experimental (NTS) group. The model of traumatic hemorrhagic shock was established after 3 hours exposure in the dry-heat environment. After the model was successfully divided into three groups according to different time points: Ti(model successfully 50 min, n = 6), T_2 (moel successfully 100 min, n = 6),T_3 (model successfully 150 min, n = 6). Normal temperature group exposed to ambient temperature 3 hours and after the model was successfully divided into five groups according to different time point: T_1 (model successfully 50 min, n = 6), T_2 (model successfully 100 min, n = 6), T_3 (model successfully 150 min, n = 6), T4(model successfully 480 min, n = 6), T5(model successfully 600 min, n = 6). To collect blood urea nitrogen and creatinine; collect renal tissue to detect inflammatory cytokines TNF-alpha and IL-1beta and the morphology change of renal glomerular and renal tubular and Paller score at each time point. Results There was no significant difference (P> 0.05) in blood urea nitrogen and creatinine between the NTS group and the DHS group at T1 time point, and there was significant difference (P< 0.05) at T_2 and T_3 time points. At T_1, T_2 and T_3 time point, Paller score, TNF-alpha and IL-1beta were significantly higher in DHS group than those in NTS group (P<0.05). Correlation analysis showed that TNF-alpha and Paller score had positive correlation (rNTS= 0.863,RDHS = 0.905, P < 0.01). IL-1beta was positively correlated with Paller score (rNTS = 0.823, RDHS = 0.946,P < 0.01). At the same time, the levels of blood urea nitrogen, creatinine, Paller score, TNF- alpha and IL-1beta in DHS group were significantly higher than those in NTS group at T3 time point (P < 0.05). Conclusion When dry-heat environment combined with traumatic hemorrhagic shock happens, the indexes of renal injury (blood urea nitrogen and creatinine), Paller score and inflammatory cytokines (TNF-alpha and IL-1beta) are significantly higher than those in traumatic hemorrhagic shock group, indicating that the dry- heat environment and traumatic hemorrhagic shock co-driven the expression of inflammatory factor may play an important role in secondary renal injury.
