沙漠干热环境下创伤失血性休克大鼠肝脏组织形态学和肝脏功能的变化特点

中文摘要目的观察沙漠干热环境下创伤失血性休克大鼠肝脏病理组织学、肝细胞超微结构及肝脏功能的变化,探讨沙漠干热环境肝损伤特点及规律。方法140只雄性SD大鼠随机分为常温环境创伤失血性休克组和干热环境创伤失血性休克组,每组又分别设对照组及休克后0 h组、0.5 h组、1 h组、1.5 h组、2 h组、3 h组7个亚组,每组10只,建立创伤失血性休克模型后,在相应时间点取材,显微镜下观察肝脏病理组织学变化,电镜下观察肝细胞超微结构变化,同时测定各时间点血清ALT、AST水平。结果创伤失血性休克后,干热环境组肝组织结构病变明显较常温环境组加重,且发生组织代偿性适应性改变时间点提前;干热环境组ALT和AST水平明显增加并呈动态变化,与肝脏组织形态病理学改变基本一致,电镜结果提示干热环境组细胞线粒体损害较常温组明显。结论沙漠干热环境下创伤失血性休克较常温环境创伤失血性休克肝脏损伤出现的更早、更严重;肝细胞线粒体损伤在肝损伤过程中可能起重要作用。
英文摘要Objective To investigate the liver pathologic histology, hepatocyte ultrastructure and the liver function in traumatic hemorrhagic shock rats under dry - heat desert environment. Methods A total of 140 male SD rats were randomly and equally divided into traumatic hemorrhagic shock group under normal environment and traumatic hemorrhagic shock group under dry - heat environment group ;and each group was equally divided into seven subgroups: control group and shock 0 h group, 0. 5 h group,1 h group, 1. 5 h group,2 h group,3 h group. The model of traumatic hemorrhagic shock was established,at the corresponding time points,rats were executed and the specimens were obtained. The change of liver pathology, hepatocyte ultrastructure under electron microscope were observed and the levels of serum ALT,AST were measured. Results After traumatic hemorrhagic shock, the lesions of liver tissue structure were more serious in dry - heat environment group than the corresponding time point of normal environment. And the tissue compensatory adaption was shift to an earlier time point in dry -heat environment group compared to the normal environment group. Under dry - heat environment, serum ALT and AST were significantly increased and appeared dynamic changes, and showed similar changing trend as the change of liver pathology morphology. Electron microscope results indicated that the damages of hepatocyte mitochondria were more serious in dry - heat environment group than those in normal environment group. Conclusion When traumatic hemorrhagic shock happens under dry - heat desert environment, the liver damage occurs earlier and more serious than the situation under normal environment. Hepatocyte mitochondria damage may play an important role in the process of liver injury.
