沙漠干热环境下创伤失血性休克大鼠血清TNF-alpha、IL-6变化与继发性肺损伤及细胞凋亡的关系研究 | |

中文摘要 | 目的探讨沙漠干热环境下创伤失血性休克大鼠血清炎症因子TNF-alpha、IL-6的变化与肺脏继发性损伤及细胞凋亡的关系及其机制。方法140只雄性SD大鼠随机分为常温环境创伤失血性休克组(常温组)和干热环境创伤失血性休克组(干热组),每组又分别设对照组、休克复苏后0、0.5、1.0、1.5、2.0、2.5、3.0h7个亚组,每组10只。建立创伤失血性休克模型后,留取血清和肺组织,ELISA法检测血清TNF-a、IL-6水平,观察肺组织病理和超微结构变化,原位末端标记法(TUNEL)测定肺组织凋亡指数。结果干热组各时间点肺损伤病理学评分均高于常温组,电镜观察发现干热组肺脏I、Ⅱ型上皮细胞水肿严重,线粒体嵴断裂、消失,血管内皮细胞水肿,有中性粒细胞浸润;干热组细胞凋亡指数各个时间点均大于常温组(P<0.05),干热组峰值出现在1.5h处,而常温组出现在2.0h处;干热组各个时间点血清TNF-alpha、IL-6水平均高于常温组,且TNF-a水平与肺损伤病理学评分、凋亡指数呈正相关(P<0.05)。结论沙漠干热环境中创伤失血性休克大鼠继发性肺损伤较常温环境下发生早且严重,细胞凋亡、血清TNF-alpha和IL-6等在继发性肺损伤过程中起到至关重要的作用。 |
英文摘要 | Objective To explore the relationship between the changes in serum inflammatory cytokines TNF-alpha and IL-6 and secondary lung injury and apoptosis in rats with traumatic hemorrhagic shock occurring in hot and dry desert environment. Methods One hundred and forty male SD rats were randomly divided into traumatic hemorrhagic shock in a normal temperature environment group (normal group) and traumatic hemorrhagic shock in a dry and hot environment group (dry-hot group). Each group was again divided into control group, 0-,0.5-, 1.0-, 1.5-, 2-, and 3-h subgroups after resuscitation of shock (n=10). The traumatic hemorrhagic shock model was reproduced. The TNF-alpha and IL-6 contents in serum were determined with enzyme-linked immunoassay (ELISA). The changes in pathology and ultrastructure of the lung tissue were observed. The lung tissue apoptosis index was assessed by TdT-mediated dUTP nick end labeling (TUNEL). Results The pathological score of lung injury in dry-hot group was higher at every time point than that in normal group. Electron microscopic studies revealed marked swelling of alveolar types I and II epithelial cells in the lungs, disruption and disappearance of mitochondrial cristae, and swelling of vascular endothelial cells, and neutrophil infiltration. The cell apoptosis index at each time point in dry-hot group was higher than those in normal group (P<0.05). All the peak value of the changes appeared at 1.5h in dry-hot group, but at 2.Oh in normal group. The serum TNF-alpha and IL-6 concentrations at each time point were all higher in dry-hot group than those in normal group. The TNF-alpha level was positively correlated with the pathological score and apoptosis index of lung injury (P<0.05). Conclusion The secondary lung injury of traumatic hemorrhagic shock in rats located in dry and hot desert is more serious, and it occurs earlier than that in normal temperature environment. The apoptosis of cells and higher TNF-alpha and IL-6 levels in serum may play an important role in the secondary pulmonary injury of traumatic hemorrhagic shock rats situated in dry and hot desert environment. |
