中文摘要	目的探讨阴囊Paget病的临床病理及免疫表型特征.方法对8例阴囊Paget病进行光镜观察、免疫组化(SP法)染色,并对其中1例做电镜检查.结果 8例Paget病变均局限于表皮层或真皮浅层,病变部位的深部组织及身体其他部位均无肿瘤存在.光镜下肿瘤细胞分别呈印戒样(2例)、大汗腺样(3例)、皮脂腺样(1例)及不规则形(2例)4种形态.肿瘤细胞分别表达CK8 (8例)、mucin-1(3例)、AR(5例),3例大汗腺样细胞共同表达GCDFP-15和AR.1例透射电镜下肿瘤细胞有分泌小管及半桥粒结构.结论阴囊Paget病为原发于表皮的腺癌,可能起源于向大汗腺、皮脂腺及黏液细胞方向或向表皮成角质细胞分化的皮肤多潜能干细胞;阴囊Paget病肿瘤细胞学分型可能有助于探讨Paget病复发、转移的规律以及新的治疗方法的研究.
英文摘要	Purpose To study the clinicnpalhnlagic and iminunohilsochemical charaijleristics of Paget’ s disease in ihe scrotum. Methods Eight oases with Paget’s disease of scrotum were analyzed hy means of elinicopathology and immunohisloehemistry ( S-P method). Results Investigations for malignancies and other invasive carcinoma in internal organs were negative in all the 8 cases. Histo-logieally,4 types of cells in Paget’s disease of scrotum were observed, including rnucin type, apoci’ine type, sebaceous type and irregularity type. Immunohistochemically, all cases expressed GK8, 3 eases expressed muciii-1 , and 5 eases expressed All. CCDFP-15 and AR were conmilanl coexpressed in 3 cases. Ullrastructurally, tin; tumor cells ol one ol the cases displayed glandular differentiation, having abundant cytoplasmic glycogen and secrete canaliculus. Conclusions Paget’ s disease of scrotum is a primary cutaneous intra-epilhelial adenocarcinoma, most likely arising from cutaneous stem cell with apocrine gland epithelial, sebaceous gland epithelial and muein epithelial differentiation. Cylological typing in patients with Paget’s disease of scrotum may aid in the research on recurrence and metastasis of the tumor and the development of a new adjuvant therapy.
