中文摘要	目的了解慢性粒细胞自血病(CML)患者外周血初始(naive)T细胞的水平和T细胞受体(TCR)Vbeta基因谱系利用和克隆性，从而了解CML患者的胸腺近期输出功能和TCR Vbeta亚家族T细胞增殖情况。方法采用实时定量PCR(TaqMan)方法检测20例CML患者外周血单个核细胞中T细胞受体重排删除环(T cell receptor rearrangement excision circles，TREC)的水平，并根据外周血中CD3阳性率计算CD3^+细胞中TREC水平。利用逆转录PCR和基因扫描分析其中14例患者外周血单个核细胞的TCR Vbeta24个亚家族基因表达和克隆性。9名正常人外周血作为对照。结果CML患者外周血TREC含量(0．060．16／1000CD3^+细胞)明显低于正常人(6．844．71／1000 CD3^+细胞，P<0．01)。14例CML患者外周血T细胞表达不同数量Vbeta亚家族(1～12个)，其中13例CML患者外周血中的一些Vbeta亚家族出现克隆性T细胞，Vbeta3，Vbeta10，Vbeta19，Vbeta21和Vbeta22亚家族的克隆性增殖多见。结论CML患者胸腺近期输出初始T细胞功能明显降低，但仍存在优势利用和克隆性增殖Vbeta亚家族T细胞，提示尽管整体T细胞免疫功能低下，但对白血病细胞相关抗原仍具有一定特异性免疫反应的能力。
英文摘要	Objective To analyze peripheral blood naive T cell level, ils T cell receptor (TCR) VB repertoire usage profile and clonalily for evaluating the recent thymic output function and the expansion feature of TCR VB subfamily T cells in palients with chronic myelogenous leukemia ( CML). Methods Quantitative detection of T-cell receptor excision DNA circles (TRl’JCs) in peripheral blood mnnonuclear cells (I’BMNC) from 20 oases ol CML was preformed by real-lime PCR (TaqMan) analysis, and TRKCs-nimiber in T-cells was calculated from peripheral blood CD3-positive cell rate. The expression and clonalily analysis were delec-ted by RT-PCR and genescan technique in PBMNC from 14 oul of the 20 patients. Nine normal individuals served as controls. Results A dramatic reduction of TRKCs value in palienls with CML was detected as com-pared with that in normal controls. The mean value ol TRECs was 0.06 0. 16 eopy/1000 CD3 * cells in CML palients while 6. 84 4. 71 copies /1000 CD3 * cells in normal controls (P <0. 01 ). The 1 - 12 Vp subfamilies were variably expressed in samples from 14 patients. Genescan analysis identified clonal expanded T cells of some Vp subfamily from 13 cases. Vp3, VplO, Vpl9, V821 and Vp22 subfamilies clonal T cells were more frequently seen. Conclusion There is a prominent reduction of recent thyinic oulpul naive T cells function in CML. The predominant usage and clonal expansion of TCR Vp subfamily T cells could be identi-fied, indicating that CML palienls have specific immune response to leukemia associated antigen, in spite ol iheir T cell immunodeficiency.
