中文摘要	目的提高肾上腺髓质脂肪瘤的诊治水平.方法回顾性分析42例肾上腺髓质脂肪瘤患者的临床资料并复习有关文献.年龄23～72岁,平均45岁.男15例,女27例.左侧18例,右侧24例.偶发24例,腰痛13例,高血压、心悸、头晕4例(24 h尿VMA增高),库兴综合征1例.病程1个月～7年.B超、CT或MRI检查,诊断髓质脂肪瘤38例.经腰部第10或11肋间径路行开放手术单纯性切除肿瘤33例,标准腹腔镜下切除肿瘤8例,手助腹腔镜下切除肿瘤1例.结果42例中,患者术后病理检查均证实为髓质脂肪瘤,其中伴腺瘤2例,伴肉芽肿性炎症1例.37例获随访,随访时间3个月～10.5年,未见肿瘤复发,4例因高血压误诊为嗜铬细胞瘤患者,术后血压均恢复正常.结论部分肾上腺髓质脂肪瘤伴有内分泌功能,影像学检查是肾上腺髓质脂肪瘤术前诊断的主要手段,开放或腹腔镜下单纯切除肿瘤为主要治疗方法,对肿瘤直径＜3.5 cm的偶发性肾上腺髓质脂肪瘤也可积极手术.
英文摘要	Objective To improve the diagnostic and therapeutic level of adrenal myelolipoma. Methods The clinical data of 42 cases (15 men and 27 women) of adrenal myelolipoma were analyzed ret-rospectively and the relevant literature were reviewed. The patients’mean age was 45 years (range, 23 -72 years). Among the 42 cases, tumors were incidentally detected in 24 cases; 13 cases had low back pain;4 had hypertension, palpitation and dizziness (increase in VMA of urine/24) and 1 had Cushing’s syndrome. Of tliem, 18 cases had the tumor on the left and 24, on the right. The disease course ranged from 1 month to 7 years. Thirty-eight cases were diagnosed of adrenal myelolipoma by B-ultrasound, CT or MRI. All the 42 ca-ses underwent surgery .including open surgical excision of the tumors in 33 cases,standard laparoscopic exci-sion in 8 , and hand-assisted laparoscopic excision in 1. Results Postoperative pathology confirmed the di-agnosis of adrenal myelolipoma in all the 42 oases (2 concomitantiy with adenoma and 1 ,with granulomatous inflammation). Of the 42 cases, 37 were followed up for 3 months to 10.5 years, and no recurrent tumor was found. In 4 patients who had been misdiagnosed to have pheochromocytoma due to hypertension, blood pres-sure returned to normal after surgery. Conclusions Some of adrenal myelolipomas are accompanied by hyperadrenalism. Diagnosis of adrenal myelolipoma can he mostly established by B-ultrasound, CT or MRI. Surgical extirpation is the primary treatment choice. The incidental adrenal myelolipoma smaller than 3. 5 cm in diameter can also be treated surgically.
