卡维地洛对心肌梗死后大鼠心肌细胞因子表达及心室重塑的影响

中文摘要目的 探讨细胞因子在心肌梗死(MI)后大鼠心室重塑中的作用,以及卡维地洛对心肌细胞因子表达和心室重塑的影响。方法 结扎大鼠左冠状动脉前降支建立MI模型,随机分为对照组和卡维地洛组,另设假手术组。卡维地洛组给予卡维地洛灌胃,对照组和假手术组仅以等体积生理盐水灌胃,观察4周末卡维地洛对心肌细胞肿瘤坏死因子-alpha(TNF-alpha)、白介素-1beta(IL-1beta)、白介素-6(IL-6)、转化生长因子beta1(TGF-beta1)和白介素-10(IL-10)mRNA的表达及心室重塑的影响。结果 MI后4周末大鼠心肌细胞因子表达(梗死区:TNF-alpha 1.070.23、IL-1 beta1.070.36、IL-6 1.250.47、TGF-beta 1.250.16、IL-10 0.840.06;非梗死区:TNF-alpha 1.050.09、IL-1、beta1.040.14、IL-6 1.040.13、TGF-beta1 1.050.07、IL-10 0.770.09)较假手术组明显增加(P〈0.01)。与对照组比较,卡维地洛组血液动力学指标及心室重塑改善,心肌致炎性细胞因子mRNA表达(梗死区:TNF-alpha 0.580.06、IL-1beta 0.710.08、IL-6 0.630.08;非梗死区TNF-alpha 0.670.19、IL-1beta 0.580.11、IL-6 0.590.15)和致纤维化性细胞因子TGF-beta1 mRNA表达(梗死区0.720.14,非梗死区0.730.16)降低,而抗炎性细胞因子IL-10 mRNA表达差异无统计学意义。结论 细胞因子可能参与了MI后心室重塑。卡维地洛改善心室重塑的作用机制至少部分与对细胞因子基因表达作用有关。[著者文摘]
英文摘要Objective To study the role of the pro-inflammatory cytokines, tumor necrosis factor (TNF)-a, interleukin (IL)-ip, IL-6, the pro-fibrosis cytokine, transforming growth factor (TGF)-^! arid anti-inflammatory cytokine, IL-10 in the process of ventricular remodeling in rat after acute myocardial infarction (AMI) and the effect of carvedilol on them. Methods Rats with AMI induced by left anterior descending coronary branch ligation were randomized to carvedilol and control groups compared with sham-operated group. The effects of 4 weeks of therapy with carvedilol starting 24 hour after myocardial infarction on myocardial cytokines mRNA expression and ventricular remodeling were studied. Results AMI caused the significant elevations of all cytokine mRNA expressions mentioned above in the infarcted and non-infarcted region at four weeks compared with sham-operated group ( all P < 0. 01) . Compared with control group, the pro-inflammatory and pro-fibrosis cytokines mRNA levels in infarcted and non-infarcted region of carvedilol group were decreased (all P < 0. 01 ) , accompanied by the amelioration of hemodynamics and ventricular remodeling(P< 0. 01 or P<0. 05). Carvedilol had no significant effect on the anti-inflammatory cytokine IL-10 mRNA expression. Conclusions Cytokines may be involved in the process of ventricular remodeling after infarction. Carvedilol reduces the pro-inflammatory and pro-fibrosis cytokines accompanied by the amelioration of ventricular remodeling. One mechanism underlying the beneficial effects of carvedilol on ventricular remodeling after infarction may be associated with the attenuation of pro-inflammatory cytokines.
