安体舒通对左旋硝基精氨酸甲酯(L-NAME)/高盐致高血压大鼠肾内小动脉重塑的影响

中文摘要目的评价醛固酮拮抗剂安体舒通能否预防左旋-硝基-精氨酸甲酯(L-NAME)/高盐高血压大鼠肾内小动脉重塑发生以及该作用在不同剂量安体舒通组是否存在差异.方法30只Spague-Dawley大鼠随机分为4组:1)对照(C)组:自来水灌胃及饮用自来水;2)NO合酶抑制剂(L)组:L-NAME灌胃+饮用1%盐水;3)小剂量安体舒通(S)组:L-NAME+安体舒通20 mg/kg?d灌胃+饮用1%盐水;4)大剂量安体舒通(B)组:L-NAME+安体舒通100 mg/kg?d灌胃+饮用1%盐水.8周后应用病理学方法测定肾内小动脉血管重塑指标.结果各组大鼠血压差异显著(F=19.799,P<0.001).C组血压明显低于L、S、B组(P<0.01),B组显著低于L、S组(P<0.05),而L、S组之间差异不显著(P>0.05).各组大鼠肾内小动脉腔径(F=6.314,P=0.003)、中层厚度(F=11.363,P<0.001)、腔面积(F=4.592,P=0.012)、中层/腔径比值(F=22.544,P<0.001)以及中层纤维化比例(F=18.729,P=0.000)显示明显的差异.表现为L组腔径、腔面积明显小于C、S、B组,而中层厚度,中层/腔径比值以及中层纤维化比例明显大于C、S、B组,而C、S、B三组间上述指标无差异.血清钾、钠、肌酐水平以及血浆醛固酮水平组间无显著差异(P>0.05).结论安体舒通均可以有效预防L-NAME/高盐致高血压大鼠肾内小动脉血管重塑发生.该作用在不同剂量安体舒通组间差异不显著,似不依赖于其对血压的降低程度.
英文摘要Objective To determine the effect of spironolactone, a nonselective aldosterone an-tagonist on renal arterioles remoderling in Ntunitro-L-arginine-methyl ester (L-NAME) and salt in-duced hypertension . Methods Thirty male SD rats were randomly received tap water group C(n - 7 ) , L-NAME (50 mg/kg·d) (group L n-1), L-NAME (50 mg/kg·d) plus spironolactone (20 mg/ kg·d) (group S n=8) , or L-NAME (50 mg/kg·d) plus high dose of spironolactone (100 mg/kg·d) (group B n=8). All rats were fed with 1% salt water except group C. Eight weeks later all the rats were sacrificed and the left kidney was separated. Plasma aldosterone level was measured. The renal rascular vessel chamber diameter (CD), area(CA), the vessel middle layer thickness(MT) and area(MA) and the ratio of middle layer thickness to chamber diameter (M/C) were calculated. The ratio of middle layer fiber area to middle layer area was determined. Results There were significant differences in the tail blood pressure among the four groups (F=19. 799, P<0. 001). Hypertension was established in group L,S,B. The blood pressure in group B were lower than that in group L arid S(P<0. 05). Significant differences were found in the CD(F=6. 314,P = 0. 003), MT(F=11.363, P<0. 001), CA(F=4. 592,P = 0. 012), M/C(F=22. 544,P<0. 001)and the fibrosis in the middle layer)F= 18. 729,P<0. 001)of the renal arterioles among.the four groups. Conclusion Spironolactone prevent the renal arterioles remoldeling in Nurnitro-L-arginine-methyl ester (L-NAME) and salt induced hypertensive rat, This favorable effect was analogous in both low and high doses of spironolactone and appeared to be independent of the blood pressure.
